I've got this idea: a tough guy–japanese SWAT (which has like, almost no publicly available information, btw), outwardly he's really macho, tall, muscular, etc, but he loves girly fashion (liz lisa, axes femme, honey, etc), and like, sanrio. He's nice and gentle in his personality, despite his appearance.

A short, cute, effete guy, who comes across as sweet and softspoken. Maybe he's a teacher or works in a cafe? But under the surface, he's a gruff asshole who's into outdoorsy stuff and guns.
I love opposites attract stories and niche japanese fashion subcultures. No idea whatsoever how a story about them would play out, so I'll need to workshop that, but its a start!

I like the whole "I got reincarnated into a character of this book/game" concept, something like SVSS or I'm In Love With the Villainess. But the problem for me with those types of story is the love interest being a fictional character, it feels a bit fake to ship them with a real person, like the relationship lacks depth. So then I was imagining a story where both the MC and love interest are real people sucked into the fictional world somehow. The MC gets reincarnated as the villain and he finds that a troublesome guy he knew in his old world is the hero(could also be vice-versa). The MC just wants to either go back or live peacefully, but the other guy is an idiot chuunibyou type who gets really into being the character and starts trying to pull some isekai protag BS and the MC has to run around getting him out of trouble.
As for their personalities, I imagine the MC as an unpopular loner, but a chill quiet one, not an incel type. As for the love interest, he has friends but he is basically the punching bag of his friend group, like the guy they keep around just to make fun of and only pretend to like. The MC is kind-hearted and feels empathy for the LI even though he's annoying and helps him out but the LI is a dick to him. Eventually though they get closer, MC saves LI from dying or something and he gradually falls in love with him+lets down his defenses and starts being more genuine. It could end either with them both going back to their IRL lives but having each other now, or leading a new life together in the game world. Not too sure on the relationship between the two pre-isekai, could be something like estranged childhood friends maybe.

Nonas, is it more interesting if a character loses an arm that has self harm scars, or if he loses the non-harmed arm instead? I am workshopping an idea, and without getting into too many details, the character in question is military and keeps a guilt tally of the people who he has killed. At some point he needs to lose an arm, and I don't know which one would be more tragic: if he loses the tally/guilt arm, then he's lost the only way he remembers the lives he's taken, and if he loses the other arm, then he only has the injured one left. I feel like the obvious answer is the tally arm, but wanted an external opinion.

I was toying with this idea of some sort of power imbalance in the 1890s between some Earl or something and his assistant. I do love me my age gaps and late Victorian stylings, and it would be very interesting (going off some discussion from a few months ago on here), if the seme could still seme without actually fucking the uke. The only real ideas I have thus far are the seme making the uke use a glory hole (as in, move his hips forward) and ye olde sybian.

His obsessive tendencies prove to be a problem and he's obviously clingy in a way that'll get MC in trouble so MC has to pacify him and give him attention without getting in trouble himself
I love your ideas for the secret professor route, I wasn't sure how it would go but a rival you have to watch out for would add such a cool layer to the game. I think he could function both as a rival or someone threatening to expose the MC. I'm having a hard time determining if the other professor is also going for students or if he's actually a moral character who wants to protect them from MC. Maybe he could pretend to have morals while secretly wanting to get MC in trouble so he can have the ukes to himself kek
The cancer thing makes for a super interesting twist at the end, I was thinking MC just got into this career in the first place with the goal of having power of others to find people to take advantage of because he's just that kind of sick person. I was actually thinking he'd been preying on students for a while, it's just he hasn't been caught yet. The inciting incident at the start of the game would be that his recent victim had killed himself and MC wants a new toy to replace him with.
Ok I ended up typing quite a bit, thanks for inspiring me to think of this again in more detail. Maybe I'll make some mspaint mock-ups of some character designs, I need to practice art more if I want to make this too. I think an under-developed artstyle is the big thing holding me back.
